What is a Geraitric Care Managment Care Plan?
By | July 25, 2013

Geriatric Care Managers (GCM’s) uses a care plan to solve older clients problems.
But what is a care plan? A care plan is a strategy to repair the holes in the older person’s web or safety net. The older person who uses a geriatric care manager is experiencing problems because the web of support or his or her own functioning has deficits or holes.
The care plan suggests a way to repair those holes by recommending the right services at the right time for the right amount of money. A geriatric care manager is like Charlotte, the crafty spider from Charlotte’s Web, using the large continuum of care in their community (like elder attorneys, plumbers, assisted living, doctors, home delivery grocery) to recommend ways to repair holes in the older person’s personal web.
The care plan is also like a prescribed remedy. The GCM identifies each problem the client is experiencing in the problem section and gives an intervention for each problem in the intervention section of the care plan. The GCM, are like a health practitioner. They examine their patient—using your psychosocial and functional assessment tools plus any other needed assessments (spiritual, dementia, etc.). You then make a diagnosis based on their assessment using the problem list in the care plan. Then, the GCM prescribes the solution to the client’s problems—the interventions or solution in the care plan.
