130 million Dead COVID- Trump will not require the #1 deterrent -Masks

July 9, 2020

Number 1 Prevention of COVID -19  Wearing A Mask

The Coronavirus yesterday reached the 3 million mark in mark in cases  The number one preventative step we could have taken is to wear masks

Citing Lone Ranger-TRUMP Refuses National Order For Masks

Yet, President Trump refuses to issue a federal order to wear masks and leaves it up to the states. In Texas, where the coronavirus rampages, the  Republican governor has also refused to issue a state order to wear masks. Like ordering the US to wear masks, has something to do with his looks, Trump said a mask makes him look like the Lone Ranger. The Lone Star state topped over 1000 cases 5 days in a row

33 Million Would Not Be Dead if Masks Ordered in  October

A new study says wearing masks would have saved 33 million lives since last October. Yet there are still no federal mask requirements.

Lack of President’s National Response Leading to New Depression- Says World Bank

This lack of federal response, like no federal order to wear masks, is not only killing huge swaths of the world population but has torpedoed our economy. President Trump wants to reboot the economy yet by not using federal controls, according to the World Bank the President is moving us to a recession and the cusp of depression worldwide.

Less than half the States Require Masks

Only 20 states require masks as Trump, instead of requiring masks, has left it up to the states. He relented recently, narcissistically,  saying he thought he looked good- like the lone ranger in a mask but ordered no federal requirement.

Dr. Anthony Fauci predicts a few days ago that the huge COVID spread could leave 100 million a day dying. Masks are our # 1 deterrent again this  slaughter by the omission of using federal powers

33 States Now in Crisis Surge of Virus Sunbelt Devastated

A total of 33 states now have a coronavirus surge with 7 states in the southwest at a crisis level.

Without required preventative steps from the Trump government and the wildfire surge immolating ¾ of our states, COVID 19 is not going away, in fact, may last well into 2021

Mixed -Messaging by President, States, Counties Gives Families no Path to Virus Safety

The problem is made more confusing by the administration’s mixed messaging with different states, cities, counties telling residents what to do to be safe. Families have no idea which way to turn to keep themselves and the most vulnerable senior family members safe.

Consult with Seniors on COVID-19 Safety Through Telehealth

Mixed Messaging to Seniors who are 80% of Deaths in COVID-19- Provide Safe Message Through Telehealth

The elderly and their families are confused about the government’s mixed message about COVID-19.Covid-19 is brutal on the elderly, with those 65 and older accounting for 80% of the U.S. deaths from data the Centers for Disease Control. The Wall Street Journal reports the same vicious statistic -80% of deaths in Europe are older people.

Each World Pandemic Picks its Victims

This is not just because older people die more than other populations. Each pandemic picks its victims. Only 13% of the estimated 12,469 U.S. deaths in the 2009-2010 H1N1 pandemic were among those 65 and older. The elderly’s apparent resistance to that virus was one reason the fatality rate was so low. The earlier H1N1 strain that caused the pandemic of 1918 and 1919 was much more virulent, causing an estimated 675,000 deaths among a U.S. population less than a third the size of today’s yet killed one-third of the world’s population. It wreaked havoc primarily of young adults.

CDC Muzzled in Telling Seniors & Citizens Real Science

The coronavirus increasing assault today is made 10 times worse by the mixed messaging in the US. The CDC, once the premier health agency in the world, is now muzzled and states that nursing home residents are at high risk to the coronavirus, where they estimate that 40-60% of all seniors will die. Yet now the CDC  states nursing homes could reopen and defers to the state law when helping the family https://www.cdc.gov/coronavirus/2019-ncov/hcp/long-term-care.html.

The present administrations mixed messaging with Covid-19, like the CDC site’s confusing directions to seniors has according to the Kaiser Foundation undermined the public trust and left seniors and their families aimlessly looking for the safest path.

Geriatric Care Manager’s and seniors services need to create their own COVID-19 Telehealth products to help seniors and their families in their community.

Create that safe path for client’s yourself

Nursing Homes residents in Pandemic -Need Help Today, Mother’s Day& Future

May 10, 2020

MOTHER’S DAY 6 FEET APART AND LONLEY

Nursing home residents, like prisoners, are locked in space “less” than 6 feet apart in facilities with a growing death rate that has reached 20% of all Covid-19. Support them on Mother’s Day and every day in the future. Their situation is increasingly dire as in Florida, a state with the largest elderly population the cases have spiked radically since restrictions were lifted  60% of new cases are in nursing homes

If you are an aging professional- step up to the plate. Through the Hartford Foundation join Huddles. The network is designed to support nursing home leadership, staff, residents, and families impacted by the COVID-19 pandemic. For the week of May 11-15, the huddles will focus on testing. They have organized the Engage Initiative for COVID-19 in Nursing Home. So join and advocate for  facilities with the highest death rate in the country- where the federal government does nothing as a holocaust is occurring

MAKE A SIGN AND VISIT A LOCAL NURSING HOME TODAY

Support older residents in nursing homes by making a SIGN and standing outside the facility. Gather some friends. People all over the country are doing this, showing the residents they are loved. In Holiday Florida, nursing home residents were treated to a mother’s day parade. In Chillicothe, Ohio. residents drove around the driveways of a nursing home in HONK for HUGS

Today or someday this week, just call a nursing home in your city and tell them what you are doing. Get their permission and ask if you can go from window to window with your sign of Happy Mother’s Day or ” whatever you wish for them “and wave. It is that simple. Today these elderly, sick and threatened mothers, cannot see anyone, often have family long distance who cannot see them, and are literally locked in a facility where beds are not 6 feet apart.

PLAY IT FORWARD – DO SOMETHING FOR NURSING HOME RESIDENTS IN THE NEXT FEW WEEKS

No matter what your state, town, or the facility is doing to protect them they are the number one target of coronaviruses doing to protects them. AARP  has started a mutual Aide group to both give and receive emotional aide during a pandemic. Join a group or organize a group to make phone calls to residents of the local nursing homes. If you are a retired medical professional down to an aide or someone who can answer the phone- instead of virtually, you could really step up to the plate, and volunteer to help a nursing home that is understaffed in the pandemic .

In my home state, CALIFORNIA, you can join the Social Bridging Program. You can also join the Friendship line to call older people who are lonely and just need you to be there to listen.

ONE SMALL STEP FOR PEOPLE IN THE BULLSSEYE OF DEATH

The point is to do something one small thing whether it is on Mother’s Day Monday or this week. Take a step to help the bullseyes targets of COVID-19 the elderly who are terrified, lonely, and now on Mother’s Day alone.

Why is CMS Loosening Nursing Homes Infection Control in Pandemic ?

May 9, 2020

When 20% of COVID-19 cases are in Nursing Homes -CMS Rolling Back Infection Control

Insanely CMS and the FEDS are considering rolling back infection control in a nursing home in the middle of a coronavirus pandemic -when more than 20 % of the cases in the US are in Nursing Homes.

The mind-boggling idea was reported in USA Today. A rule proposed last year by the Centers for Medicare and Medicaid Services (CMS) would modify the amount of time infection prevention must devote to a facility from at least part-time to “sufficient time,” an undefined term that lets the facility decide how much time should be spent.

Why do you need infection control in a nursing home- even before COVID -19?

Over 4 million Americans are admitted to or reside in nursing homes and skilled nursing facilities each year and nearly one million persons reside in assisted living facilities. Data about infections that occur in these skilled nursing home include on a normal day before coronavirus

  • 1 to 3 million serious infections occur every year in these facilities.
  • Infections include urinary tract infection, diarrheal diseases, antibiotic-resistant staph infections, and many others.
  • Infections are a major cause of hospitalization and death; as many as 380,000 people die of the infections in LTCFs every year.
  •

Nursing Homes deaths May Be 50% of All Deaths in Pandemic

These infection control rationals were before COVID-19. Now, in the midst of this black plague of the 21st Century, it is thought, according to the Washington Post that nursing homes may account for 50% of death BEFORE infection control is further diluted by CMS.

Cities& States Reduce COVID-19 Risk in Nursing Homes -Feds Increase Them

In contrast to the blighted policy idea of the Federal government, Detroit, as a city, has tested every nursing home and in addition, is testing all staff and providing them with PPE.San Francisco is also now testing every resident of a nursing home. States like Maryland and Wisconsin and testing all nursing homes.

The Kaiser Family Foundation reports that at the end of the pandemic we may find out half of the deaths in the pandemic were from residents of nursing homes. In light of all of this carnage and potential destruction of the elder population in the US- why then are  CMS and Trump loosening infection control? Is this throwing salt into the would of frail elders, madness, an attempt to delete people on social security, incompetence at CMS or CMS following the bedeviled destructive policy of the Trump administration? We need answers but will we as a country get them in time?

Advanced Directives AND COVID-19- Do You Know When To Initiate in the Pandemic

May 5, 2020

Covid-19 Makes Signing Advanced Directives Critical

Advanced directives are even more critical with COVID-19 patients, who are often elders. The New York Times had a story about an older woman who was suffering from Covid-19 When she entered the hospital no one asked about advanced directives. After several weeks on a ventilator, the hospital asked about advanced directives, she had none. All her elderly sisters could recall was what she had said about dying when she was young.

WHEN DO YOU DISCUSS ADVANCE DIRECTIVES?

Advanced Directives should be part of your initial psychosocial assessment. Once the COVID-19 diagnosis is known with an elderly client, the care manager who has added end of life services to their agency is often the one who will initiate and guide advance care planning discussions. As difficult as these discussions may be, the burden on the family is significantly lessened if decisions about advance care planning are made before the client’s condition worsens.

Hopefully, this has already been done but many people put it off for fear of death. A recent study found that less than 50% of severely or terminally ill patients had an advance directive in their medical record.

Advance Directives

Advance directives are legal documents that allow clients to make decisions about their health care and finances in advance of when they are not mentally or physically able to do so. These documents must be signed, dated, and witnessed naming another person to make decisions for you.

Your job as a care manager is the make sure the dying client has these documents:

• A durable power for an attorney for healthcare

• A living will

• A do not resuscitate order DNR (efforts to restart the heart after it has stopped

If the client does not have these legal documents and wishes to create

them, the Geriatric Care Manager will suggest that the documents be put in place with the oversight and consultation of an elder law attorney. But if hospitalization is imminent, getting the patient to consider signing with legal consultation maybe your own option, with the family’s permission.
