 A Care Manager Can Navigate to a Good Death

Create an end-of-life care management service that provides a service in the 5 stages of death. Every stage of dying can be part of a good death, including the difficult point called the chronic phase if a care manager is a navigator for the dying person and their family.

The chronic stage of death is the time of loss

The Chronic stage is the time between the diagnosis and the result of treatment. During the phase, the dying person tries to cope with the demands of daily life while also going through necessary medical treatment, “often having to struggle with the sometimes brutal side effects of their treatment”.Chronic illness may also involve repeated episodes of deterioration in which the patient confronts and adjusts to these losses. Examples of these losses include cognitive function, sexuality, toileting, the ability to ambulate, eat and dress, and the indignity of losing all your hair. The focus of life for both the family and the patient needs to be redefined, shifting from hope for a cure to coping with the illness

Geriatric care manager tasks:

  1. Assist family to determine the type of long-term care which may  be safest and healthiest for the loved one: institutional: hospital chronic care or nursing home care, in-home nursing care or family care and make arrangements
  2. Co-ordinate help from community organizations through the continuum of care
  3. Assist client and family connect with support groups in death and dying
  4. Assist in learning management of disease skills such as from health care staff, videos, manuals, or brochures.
  5. Monitor anticipatory grief needs
  6. Learn about the disease in order to help the patient make good decisions about his/her care and to help family members monitor their expectations
  7. Monitor caregiver burden: encourage family caregivers to take time for selves, take breaks, get rest get to medical appointments, for grief needs
  8. Assess client’s non-medical needs: transportation, physician’s appointments, household tasks, personal care  if hospice  involved- medical if not involved
  9. Assess family caregiver for overload, burnout, educational supports, home care supplement or family replacement care

Dysfunctional Families and Anger – physical anger, emotional abuse, financial abuse

Fiscal and emotional abuse are the red blinking second signs of the dysfunctional aging family. Anger occurs normally in all families. However, in dysfunctional families, anger may take the form of repeated physical or

emotional abuse. This happens not only in one generation but passed on to the next and perhaps all subsequent generations. Adult children are often getting even with their aging parents for being neglectful, perhaps physically abusive parents when these angry adult children were kids.

Fiscal abuse is a ” sleeper” form of abuse,  yet a pernicious aging family problem. Adult children committed a whopping 40% of the abuse. Other family members were the next big thief’s investigated. The coming holidays create jaw-clenching stress on adult children who visit and finally see their parents decline. This means they need to care for them, which they have no idea how to do. Part of that comes from- they were never cared for as kids themselves. The holidays can be saved for a dysfunctional family, by care managers. Care managers can help them avoid abuse and support them to get the care they need for their aging parents who neglected them as children. 

Financial Advisors Can be Thieves Too

President Obama’s administration attempted to protect investors, who are often people trying to plan their retirement. The so-called fiduciary rule would have required financial advisers working with retirement accounts to put the interests of their clients ahead of their own—

The Trump administration, aiding and abetting in this theft, “blocked this rule and continues to block many retiring and aging client interests – putting them second and financial advisors interest (or ) profit first. This rule was supposed to go into effect on January 1, 2018, but was blocked again by the Trump Administration.

The securities industry received an early Christmas present from Trump’s outgoing Administration when the White House hastily cleared the way for the Department of Labor to lower the standard of care of investment advice fiduciaries, and this represents a dramatic and unprecedented reversal of the intent of Congress in enacting ERISA. The Public Investors Advocate Bar Association stated that “Workers and retirees deserve far better. They deserve to have their retirement savings protected, and they should be able to rely on those they have gone to for investment advice.”

Trump’s COVID Mixed Messages Push-Up-Deaths So No CLear Path for AGING Families

The World Health Chief warned that in a Pandemic mixed messages coming from national leaders give their people no clear path to follow to keep themselves safe from COVID. This is esspecially true of aging family members who are trying to protect the most vulnerable to coronavirus-  older people.

 

In US Federal Government and US Scientist Point to two Different to paths to” Safety”

In the US Federal officials, like the TRUMP administration offers a murky path to stay safe -while top scientists like the CDC and Dr. Fauci offer science-based recommendations. It is like a signpost is a tsunami zone with two different directions to higher ground. State governors who have been left in charge of protecting American citizens, rather than the federal governments and all have different messages. Some states are closed down, while others do not allow masks in spite of the massive spread of the virus. People do not know what to do, as the Feds, the states, the counties, and cities all have different guidelines and laws about how to stay protected from the worst pandemic since 1918.

President Woodrow Wilson Pointed the Wrong Direction is 1918 Pandemic

It is not new. President Wilson lied to the American public about the 1918 pandemic as he had declared war with Germany and wanted the US war machine to go full throttle. So soldiers in camps in the US died by the thousands and sailed to Europe spreading the 1918 plague, while Wilson never uttered a word, except to say it was like the common cold. He contracted COVID in France, trying to negotiate the Treaty of Versailles, subsequently had a stroke kept secret,(stroke being a side effect of COVID we now know ) and his wife effectively ran the county.

Science is being Maligned and Sidelined in COVID-19

What happens these when confusing messages are given by so many leaders who are not scientists but politicians with their own agenda? Scientists like Dr. Fauci are both maligned and sidelined. .The CDC once the most prestigious health agency in the world is forced to give false scientific information by the TRUMP administration.

 One terrifying result is COVID cases are soaring in the US As of  September 3, we had 6.1 M cases and 189 K deaths.  Younger people and even older residents are celebrating summer and now Labor Day with no masks no social distancing as if there was no COVID. We now celebrate Labor day with people gathering with no masks, college opening with the spread of the virus, and children used as guinea pigs in some states mandated to return the schools that have no federal money to make their classrooms safe 

Aging families, who want to keep their loved ones safe need aging professionals to offer clear paths to safety. Can you be that navigator to safety?
