Concierge Clients, wanting concierge care providers, are sadly the only clients of GCM or ALCA experts because of income inequity in the US and the fact that Medicare does not cover long-term care,  like home care and geriatric care management.

You Are Only As Strong As Your Weakest Link Especially With Concierge Clients!

 You must serve the top 10%  of aging Americans because those clients make your ALCA business or GCM  business make money and thrive. But to deliver the gold standard services that concierge care demands, let’s take the weakest link in your business- your care staff. You are only as strong as your weakest link these concierge care providers. These incredibly important employees’ skills will measure how concierge clients really rate you.

First and foremost to serve Concierge clients you need highly skilled concierge care providers with a CNA background or years of experience. Older generations, especially entitled clients, are likely to be

less trustful of an individual with numerous visible tattoos and a Mohawk. They would rather have their concierge care providers be a clean-cut employee wearing a uniform. You want background-checked employees with tolerant of demanding clients and willing to be servile and assertive and know when to use either.

Passover Begins Friday-Care Managers Can Support Clients Spiritual Needs

Passover begins Friday and brings many Jews will celebrate with a seder

where their spiritual beliefs will be celebrated with food, family, and ritual.

As we age joining in spiritual rituals means more and more to us. Ritual

religious holidays like Passover fill all faiths, but especially elders faiths and both spiritual and emotional needs.

A Care Manager needs to learn how to offer elders support and inclusion during  holidays like Passover or any religious holiday.

Spiritual rituals mean more as we age

Care management is first and foremost a holistic interaction between

a caring professional and an older adult. Holistic care works

with the senior beyond just the necessary formal services. Care

managers should work with the seniors and their families to know their religious needs if any. Getting to know the person includes understanding the physical, social, emotional, and spiritual needs and how you can help them get included in the celebration.

 

Even though Passover begins Friday the average care manager does not see themselves as doing therapy with their clients or encouraging religion. But the work of Carl Jung or Viktor Frankl may be more than is useful. For most care managers , it is helpful to follow a few rules:

1. Listen to the client. Understand the client’s perception of the religious phenomenon as the person describes it and how you can help them celebrate Passover or any religious holiday if they wish.

2. Listen to his or her the perceptions of the client’s faith tradition and

spiritual beliefs and how much and how they want to be included in family celebrations or religious services so you can help them do this, through contacting the family, the spiritual place of worship, so they can attend and have transportation or  arrange some form of celebration if homebound

3. Consult with the Rabbi from the Temple near them, who can help interpret any

beliefs or rituals or symbols that cannot be fully understood from the description of the client and if the temple has transportation for elders to services if the client wishes to attend.

What Happens When High Priestess of the Holidays Falls off her Throne?

Mothers are the high priestess of the ritual- like Queen Elizabeth without servants.

This sets up a filial crisis as women age. The UK estimated that there are 25 to-do’s women have on the holiday. It takes years to accumulate objects ritual dishes and religious objects used. It takes the left side of your brain executive skills, plans and organize, remember details, does things based on your experience. This eventually leads to a filial crisis and passing the torch.

Holidays are often done on autopilot

Women as they age –recalling all the jobs that must be done year after year get worn down in their assigned role as ” Mothers are the high priestess of the ritual.”

 It also takes their IADLs- (Instrumental Activities of Daily Living) meal planning, shopping, plus ambulation. Then add depression – widowhood, loss and you have the challenges of aging in managing these entire titanic rituals mothers are assigned by society.  Many times the aging Mom can no longer balance all these plates and the holidays shatter with the crashing ritual dishes.

When Mom Cannot do all the Holiday Preparation a Filial Crisis Occurs

 

Then the torch has to be passed and an adult child (usually an adult daughter must take over to resolve this filial crisis. This passing the torch is like secession, – Princess Elizabeth taking over for her Dad, King George, (who hated it and had a lifelong stutter) made famous in The King’s Speech who was handed the throne by his brother Edward who quit being king.   

WhenMom needs to Pass the Torch-Some  Baby Boomers Kids Shocked

Baby boomer-adult children and aging parents are unprepared by their own culture for this new developmental phase of passing the torch. They do not expect it as they did

the nights of the crying newborn or the rebellious teen, and are thrown off balance by the sometimes sudden and usually unexpected loss of their anchoring aging parents, when they find elderly mom is unable to pull off running the holidays  Indeed, what must happen in this new developmental phase is that the adult child must evolve beyond the needy child, he or she has been, depending on his or her parents for that fiscal, emotional, social support and ritual organizing parents, like managing the officiating over the Christmas or Hanukkah celebration and take on filial responsibility to avoid a filial crisis.

 Geriatric Care Manager to the Rescue

In the normal healthy family system this filial crisis of Holiday rituals can be overcome and the adult children with the brief help of an aging life or geriatric care manager they can let go of their former dependent roles and confront their parent’s loss by organizing and providing care. They can take over Christmas and Hanukkah by stepping in and grabbing that torch.

Geriatric care managers understand that the adult child must transition to what social work pioneer Margaret Blenkner labeled the filial crisis to filial maturity or a new mature state where they, as midlife adults, can give up their former roles as dependent, needy children and start to provide care to their old/old parents.

Dysfunctional Family Do Not Want to Take Over for Mom

In the dysfunctional aging family, this filial crisis is incredibly hard to trounce from both the parents’ and the adult child’s point of view. They really need a geriatric care manager’s services

Why are Care Providers for Clients So Hard to Find ?

Home care aides all over the US are AWOL. Care Management, home care agencies, nursing homes are severely understaffed and cannot find enough caregivers to staff cases for shifts. Why?

Disorganization of Home Care Industry

Staffing shortages have frustrated the home care and care management industry for

decades, according to Paula Hahn author of the New old Aging blog for the New York Times. . There is no national recruiting or training program for this huge industry.The home care industry has always been a hodgepodge of for-profit companies and chains, nonprofit programs and publicly funded care through Medicaid, all operating under a confusing welter of state and federal regulations, plus an uncharted “gray market” of clients who avoid agencies and hire privately. But Covid-19 has intensified the problem.

“I’ve never heard such frustration over finding workers, and I’ve been doing this for 20 years,” said Vicki Hoak, executive director of the Home Care Association of America, whose 4,000-member agencies collectively employ about 500,000 people. 

Bureau of Labor Predicted this in 2020 -Pre-COVID

This was foreseen and was by the Bureau of Labor statistics. They predicted in 2020 that the long-term care sector would need to fill an additional 6.2 million direct care job openings between 2019 and 2029 as workers leave the field for a new occupation or leave the labor force altogether due to retirement, disability, or some other reasons. Combining these departures and new jobs, they projected a 7.4 million total direct care job openings in the decade ahead 

Mammoth Growth in # of Seniors Who Want to Age

The bigger problem is the growing need for home care. Each day 10,000 seniors turn 65

in the US and the number of older adults will more than double over the next several decades to top 88 million people and represent over 20 percent of the population by 2050. . The majority want to age in their home and need home care, where today there are not enough home care workers to fill those jobs.

More $ Spent on Home Care than Nursing Homes

2015, was the first time nationally that more money was spent on home care than nursing home care. We’ve seen a culture and financing shift toward home and community-based care.”
Covid -19 now the Delta variant has wreaked more chaos into the home care market. Home care workers are mainly women and had to stay home with their children when school closed down. They were also hot hard by COVID themselves, working with seniors before a vaccine was developed.

What is President Biden’s New Infrastructre Caregiver Plan?

President Biden announced the American Jobs Plan, a $2 trillion investment in infrastructure, jobs, and home care. It includes a $400 billion investment to expand access to Medicaid home and community-based services (HCBS)

Why This Plan for Home-Based Care?

Only 15% of U.S. seniors 80 or older receive care at home. In Switzerland, Denmark, Mexico, and Sweden, that figure is above 30%, with numbers even higher in Israel and Lithuania.

Medicaid has slowly evolved over the years, placing a greater emphasis on home- and community-based services. But the ground gained has been relatively minor compared to investments in other settings like nursing homes.

In 1995, Medicaid spent about 18 cents out of every long-term care dollar on HCBS services. Today, that number has reached 57 cents per dollar.

The Scale of the Problem is Mamouth

The number of seniors is projected to grow by more than 40 million, approximately

doubling, by 2050, while the population older than 85 will nearly triple. Unlike most other industrialized nations, the United States does not provide a public long-term-care benefit for all older adults.

Only10-15% of Americans can Afford Private Duty Homecare

Because Medicare does not pay for home care, only the top 10-15 % can afford private duty home care, while in many other nations it is a right and free to all through the government. What Biden’s new plan does is take a giant step in making home care possible a greater number of Americans.

The plan calls for expanding access to and quality of HCBS to help more older adults and people with disabilities live in the community and extending the Money Follows the Person program to help individuals who are in nursing facilities and other institutions return to the community. 

What The Pandemic Has taught us about Aging and Dying

at Home

What the continuing pandemic has painfully taught us is when we are ill- we want to be home in the arms of our family. When we are threatened with or facing death, we do not want want to be in a sterile hospital with only anonymous caring nurses and zoom to comfort us in our last minutes in this world. We want to be at home- in the arms of our family and loving caregivers.

Long before the COVID-19 emergency, health care policy experts have increasingly recognized the value of home-based health care. A recent AARP survey found that three in four adults 50 years and older would prefer to age in their homes and communities. And a growing body of evidence suggests it is less expensive to deliver care in the home.

Indeed, for years we’ve seen hospitalized patients more quickly returning to their homes and communities to heal and recover safely, reducing costs for themselves and the health care system.

The Plague of Isolation on top of the Plague of COVID -in Locked Down Nursing Homes

Home-based care addresses negative health effects of social isolation and loneliness, which drive poorer health outcomes that annually cost billions of excess health care dollars. 

Isolation is also equal to 15 cigarettes each day. Elders in a nursing home during the pandemic were isolated from the families and their fellow residents so much so that despair may have led to the 174000 deaths as of March 2021. 

So Biden has launched a bill the builds the infrastructure of home care for the lower 30% on Medicaid until in America’s future, Medicare and home care for all is reached.
