Make Interventions Measurable with a Timeline.Your intervention should have measurable goals based on a timeline. For example, the problem of resistance to care: Mrs. Sterling has resisted care because she wanted her daughter to do the care. Intervention:
a. The GCM will visit weekly for 1 time a week to help Mrs. Sterling accept Acme private duty care provider and monitor her adjustment to new care provider.
b. Daughter Jane will make social visits three times a week to spend 1 hour with her mother
c. Daughter Jane will take her mother to Glittery Nails once a month so they can both get their nails done together.
