To create a care plan , a geriatric care management, you are sorting through lots of information you gathered. The next step is to sort through these data, like Sherlock Holmes.
This is where you sift through clues, leaving no stone unturned, to ensure that all evidence has been taken into account before reaching conclusions. You must examine the clues closely. Review every assessment – psychosocial, functional, depression, home safety, caregiver stress, quality of life- mini mental- whatever you have chosen from your care management toolbox.
Talk to each person in the formal or informal support system and make sure all evidence is taken into account.
Everyone has a different version of what happened to the older person. Analyze the client’s problems from all perspectives and then synthesize all the opinions into one truth, which will form your professional opinion. Collect all the data, and then look at each person’s point of view to come up with your own professional GCM point of view.
